I am aware that during the carving competition or any other program.
I am participating in here that certain damages, accidents or injuries may occur because of the hazards of the endeavor.
I have confirmed that my insurance covers injuries that happened overseas.
In consideration of this, I have and do hereby assume all of the above mentioned risk and all responsibilities for any occurrences.
I accept responsibility for any and all liabilities, actions, causes of action, debts, claims, demands of every kind and nature what-so-ever which may arise from or in connection with this Competition.
The right to use my portrait will belong to Toei town office.
The terms hereof shall serve as a release and assumption of risks for myself, my heirs, executor, administrators, and for all members of my family, including any minor accompanying me.
